Research Objectives The primary objectives of this study at Merowe Medical City (MMC) are: 1. To assess patients’ knowledge regarding radiotherapy, specifically focusing on its clinical purpose, radiation safety, treatment procedures, and the management of side effects. 2. To evaluate patient attitudes, encompassing their levels of fear, trust in medical staff, and general acceptance of radiation as a treatment modality. 3. To identify perceived barriers to accessing and completing treatment by utilizing the 5-A access framework (Availability, Accessibility, Affordability, Accommodation, and Acceptability). 4. To explore patient experiences and qualitative suggestions through the "Patient Voice" to identify areas for service improvement. 5. To examine associations between knowledge, attitudes, and barriers with specific sociodemographic and clinical characteristics, such as education, income, and residence. Methodology The study employs a rigorous cross-sectional, descriptive-analytical design: • Setting & Participants: The research will be conducted at MMC, targeting adult oncology patients who are currently undergoing or have been recommended for radiotherapy. • Data Collection Tool: A structured, five-part questionnaire will be used, covering: (1) Sociodemographic/Medical Background, (2) Knowledge, (3) Attitudes, (4) Perceived Barriers, and (5) Open-ended Patient Voice. • Statistical Analysis (SPSS v26.0): • Descriptive Statistics: Categorical data will be presented as frequencies and percentages, while continuous scores will be reported as mean ± standard deviation or median with interquartile range (IQR). • Inferential Testing: Chi-square tests will assess associations between categorical variables, and Independent t-tests or one-way ANOVA will compare mean scores across groups. • Correlation & Regression: Spearman’s rank correlation will evaluate the relationship between knowledge and attitude scores. Simple Logistic regression will identify independent predictors for high-level barriers. • Qualitative Analysis: Open-ended responses will undergo thematic analysis to identify recurring themes regarding patient satisfaction and needs. • Note: Following the supervisor’s recommendation, Path Analysis has been excluded from this statistical plan. Expected Results Based on established literature in similar clinical settings, the study anticipates the following outcomes: • Technical Knowledge Deficits: A high prevalence of technical misconceptions, particularly regarding radiation dermatitis and safety protocols. • Predominance of Logistical Barriers: Given the geographic context of the Northern State, travel distance, transportation costs, and accommodation needs are expected to be the most significant obstacles. • Psychological Barriers: "Fear of the unknown" and radiation-related anxiety will likely emerge as significant deterrents to treatment acceptance. • Socioeconomic Influence: Higher levels of education and income are expected to correlate positively with health literacy and lower perceived barriers. • Clinical Justification: The results will likely demonstrate that structured pre-treatment education and improved communication are vital for reducing patient anxiety and enhancing clinical adherence at MMC.
